Every how many minutes to check fetal pulse with second stage of labor Every ?
Explanation
Core Concept: The second stage of labor (from full cervical dilation to delivery) involves intense maternal pushing and significant fetal head compression, increasing the risk of fetal hypoxia and cord compression. Frequent fetal heart rate (FHR) monitoring is critical to detect distress early.
Clinical Presentation:
- Maternal bearing-down efforts.
- Potential FHR abnormalities such as severe variable decelerations (cord compression) or late decelerations (uteroplacental insufficiency).
Diagnosis:
- First-line diagnostic plan: Intermittent auscultation of the FHR every 5 minutes during the active second stage (or continuous electronic fetal monitoring if high-risk).
- Most accurate (gold standard) test: Continuous electronic fetal monitoring (EFM) or fetal scalp electrode for precise, beat-to-beat variability assessment.
Management:
- Acute management: If non-reassuring FHR patterns occur, initiate intrauterine resuscitation (maternal repositioning, IV fluid bolus, supplemental oxygen, and cessation of oxytocin).
- Gold standard long-term management: Expedited operative delivery (vacuum, forceps, or cesarean section) if fetal distress persists and delivery is not imminent.
