  • Tachycardia, hypotension, pallor, and dizziness.
  • Complications include hypovolemic shock, multi-organ failure, and death.

Diagnosis:

  • First-line diagnostic tool: Frequent vital sign monitoring (every 15 minutes), fundal palpation to ensure it remains firm and midline, and visual inspection of the perineum and lochia.
  • Gold standard diagnostic test: Quantitative blood loss (QBL) measurement, CBC to check hemoglobin/hematocrit drop, and bedside ultrasound if a concealed hematoma or retained tissue is suspected.

Management:

  • Acute management: Uterine massage, administration of prophylactic or therapeutic uterotonics, IV fluid resuscitation, and surgical repair of any identified lacerations or evacuation of hematomas.
  • Gold standard long-term/definitive management: Continuous monitoring until hemodynamic stability is assured, followed by routine postpartum care focusing on maternal-infant bonding, breastfeeding initiation, and discharge planning.
