Core Concept: Immediately after the delivery of the baby and placenta, the uterus begins the process of involution. At term, the pregnant uterus weighs approximately 1000 grams (1 kg) and its fundus is palpable at the level of the umbilicus. It will gradually shrink to about 500g by one week postpartum, and return to its non-pregnant weight (50-100g) by 6 weeks.

Clinical Presentation:

  • Symptoms: Abdominal cramping (afterpains), especially during breastfeeding due to oxytocin release.
  • Complications: Subinvolution or uterine atony leading to postpartum hemorrhage.

Diagnosis:

  • First-line/Gold Standard: Clinical palpation of the uterine fundus to assess its height, consistency (firm vs. boggy), and position.

Management:

  • Acute: Encourage frequent breastfeeding and administer uterotonics (e.g., oxytocin) if the uterus is boggy.
  • Definitive: Routine postpartum assessment to ensure normal involution over the 6-week puerperium.
