Core Concept: Group B Streptococcus (GBS) is a common bacterium that colonizes the maternal gastrointestinal and genital tracts. While usually harmless to the mother, it can be transmitted to the neonate during vaginal delivery, leading to severe early-onset neonatal sepsis, pneumonia, or meningitis.

Clinical Presentation:

  • Symptoms: Asymptomatic in the vast majority of pregnant women.
  • Complications: Neonatal sepsis, meningitis, pneumonia, and maternal chorioamnionitis.

Diagnosis:

  • First-line/Gold Standard: A combined vaginal-rectal swab for GBS culture performed between 36 and 38 weeks of gestation.

Management:

  • Acute: Administer intrapartum antibiotic prophylaxis (IV Penicillin G or Ampicillin) during active labor or upon rupture of membranes for GBS-positive mothers.
  • Definitive: Close observation of the neonate for signs of infection for at least 48 hours post-delivery.
