Core Concept: ASC-US is the most common abnormal Pap smear result, indicating mild cellular changes that are not clearly precancerous but are not entirely normal. It is often caused by transient HPV infection, inflammation, or atrophy.
Clinical Presentation:
- Asymptomatic; found on routine cervical cancer screening.
Diagnosis:
- First-line & Gold Standard: According to ASCCP guidelines, the preferred next step for ASC-US is reflex HPV DNA testing to triage the patient.
Management:
- Acute: If the HPV test is positive (especially for high-risk strains like 16/18), the patient is referred for colposcopy. If the HPV test is negative, the risk of high-grade dysplasia is extremely low, and the patient can return to routine screening intervals.
- Long-term: Routine adherence to age-based cervical cancer screening guidelines.
