Core Concept: Cyclothymic disorder (cyclothymia) is a chronic, fluctuating mood disorder characterized by numerous periods of hypomanic symptoms and numerous periods of depressive symptoms. For a diagnosis, these symptoms must persist for at least two years (one year in children/adolescents) and never meet the full criteria for a major depressive, manic, or full hypomanic episode.
Clinical Presentation:
- Chronic mood instability, periods of elevated mood, increased energy, and decreased need for sleep alternating with periods of low mood, fatigue, and hopelessness.
- Symptoms are distressing but generally not severe enough to cause marked impairment in social or occupational functioning (unlike Bipolar I or II).
- Complications include progression to Bipolar I or II disorder and substance abuse.
Diagnosis:
- First-line diagnostic tool: Detailed psychiatric history and mood charting to establish the chronicity (at least 2 years) and sub-threshold nature of the mood swings.
- Gold standard diagnostic test: Comprehensive clinical interview using DSM-5 criteria, ruling out substance-induced mood disorders or general medical conditions (e.g., hyperthyroidism).
Management:
- Acute management: Psychoeducation about the illness, establishing a regular sleep-wake cycle, and monitoring for escalation to full mania or severe depression.
- Gold standard long-term/definitive management: Psychotherapy (CBT, interpersonal and social rhythm therapy) and mood stabilizers (e.g., lithium, valproate, or lamotrigine) if symptoms cause significant functional impairment.
