Core Concept: Postpartum blues is a mild, transient mood disturbance occurring in up to 80% of women in the first 1-2 weeks after delivery. It is characterized by tearfulness, mood lability, and anxiety, and resolves spontaneously.

Clinical Presentation:

  • Tearfulness, irritability, anxiety, fatigue, and mood swings peaking around days 4-5 and resolving by 2 weeks.

Diagnosis:

  • First-line tool: Clinical history and observation.
  • Gold standard: Differentiation from postpartum depression (which is more severe, persistent, and impairs function) using screening tools like the Edinburgh Postnatal Depression Scale (EPDS).

Management:

  • Acute: Reassurance, education, and emotional support.
  • Long-term/Definitive: Encourage family support, adequate rest, and follow-up to ensure symptoms do not progress to postpartum depression. Pharmacotherapy (SSRIs) is not indicated for blues.
