Clinical Presentation:

  • Fetal macrosomia, meconium-stained amniotic fluid, and decreased amniotic fluid volume.
  • Maternal risks: Prolonged labor, operative vaginal delivery, severe perineal lacerations, and Cesarean delivery.

Diagnosis:

  • First-line: Accurate dating via first-trimester ultrasound and reliable last menstrual period (LMP).
  • Gold Standard: Serial ultrasounds and antenatal fetal surveillance (NST/BPP) if pregnancy extends beyond 41 weeks.

Management:

  • Acute: Induction of labor is typically offered between 41 and 42 weeks to prevent post-term complications.
  • Long-term/Definitive: Cesarean Section (C/S) is the most common maternal operative risk encountered due to labor dystocia, cephalopelvic disproportion from macrosomia, or non-reassuring fetal status.
