A 72 years old male known to have hypertension and is heavy smoker was found incidentally to have abdominal pulsatile mass. Ultrasound abdomen: 4.5cm abdominal aortic aneurysm. Which of the following is the best management?

Core Concept: Abdominal aortic aneurysms (AAA) are focal dilations of the aorta. The risk of rupture correlates with the diameter. Management depends on the size and growth rate.
Clinical Presentation:
- Often asymptomatic and discovered incidentally on imaging
- A pulsatile abdominal mass may be palpable in thin patients. Rupture presents with severe abdominal/back pain and hypotension
Diagnosis:
- First-line diagnostic plan: Abdominal ultrasound for screening and surveillance
- Gold standard: CT angiography (CTA) for preoperative planning or if rupture is suspected
Management:
- Acute management: For an asymptomatic AAA measuring 4.5 cm in a man, the risk of rupture is low
- Gold standard long-term management: Surveillance with regular ultrasound imaging (e.g., every 6-12 months) and strict cardiovascular risk factor modification (smoking cessation, BP control). Surgical or endovascular repair is indicated when the AAA reaches >= 5.5 cm in men, >= 5.0 cm in women, or expands rapidly (>0.5 cm in 6 months).
