Explanation
Core Concept: Women with epilepsy on antiepileptic drugs (AEDs) have a higher baseline risk of having offspring with neural tube defects (NTDs). High-dose folic acid (4-5 mg) is strictly indicated for women with a *personal* history of an NTD-affected pregnancy or those on specific high-risk AEDs like valproate.
Clinical Presentation:
- Preconception counseling for a woman on lamotrigine with a distant family history (cousin) of spina bifida.
Diagnosis:
- N/A (Preventive Obstetrics).
Management:
- Acute/Long-term: A cousin's history does not elevate the patient to the high-risk category requiring 5 mg of folic acid. Therefore, the standard recommendation for women on most AEDs without a personal history of NTDs is a prenatal multivitamin containing at least 400 mcg (0.4 mg) to 1 mg of folic acid, started preconceptionally to reduce the risk of NTDs.
