A child with Jaundice and treat with photography , how can you protect the child skin?
Explanation
Core Concept: Neonatal phototherapy uses high-intensity light to break down unconjugated bilirubin in the skin. Skin integrity must be maintained, and certain substances can cause thermal injury or block the therapeutic light.
Clinical Presentation: Neonatal jaundice (yellowing of sclera and skin), lethargy, poor feeding in severe cases.
Diagnosis: Total serum bilirubin (TSB) levels plotted on an age-specific nomogram (e.g., Bhutani).
Management: Acute: Maximize skin exposure to light. Avoid oily substances, lotions, or ointments (like Vaseline) on the skin, as they can absorb heat from the phototherapy lights and cause severe thermal burns, or act as a physical barrier blocking the light. Long-term: Monitor hydration, ensure frequent enteral feeding to promote bilirubin excretion via stool, and monitor for rebound hyperbilirubinemia after stopping therapy.
