Clinical Presentation:

• Minor, clean wound in a patient whose last tetanus toxoid-containing vaccine was exactly 10 years ago.

Diagnosis:

• Clinical assessment of wound severity (clean/minor vs. contaminated/severe) and verification of vaccination records.

Management:

• For a clean, minor wound, a tetanus booster is indicated if it has been ≥ 10 years since the last dose. For severe or contaminated wounds, a booster is given if it has been ≥ 5 years.
• Since she is due for a booster, the preferred vaccine for adults who have not previously received it is Tdap (Tetanus, Diphtheria, and acellular Pertussis) to provide pertussis immunity, followed by Td or Tdap every 10 years. Tetanus Immune Globulin (TIG) is not required for minor wounds in fully immunized individuals.
