الشرح

Core Concept: Arterial Blood Gas (ABG) interpretation involves analyzing pH, PaCO2 (respiratory component), and HCO3 (metabolic component). A pH < 7.35 indicates acidosis. An elevated PaCO2 (> 6.0 kPa or 45 mmHg) indicates a respiratory cause.

Clinical Presentation: Hypoventilation, hypoxia, confusion, lethargy, or depressed CNS function due to CO2 narcosis in a mechanically ventilated patient.

Diagnosis: First-line/Gold Standard: ABG analysis. Here, pH 7.16 (acidosis), PCO2 10.66 kPa (~80 mmHg, high), HCO3 24 (normal). This indicates uncompensated respiratory acidosis.

Management: Acute: Improve alveolar ventilation by adjusting mechanical ventilator settings (e.g., increasing respiratory rate or tidal volume) or clearing the airway. Long-term: Treat the underlying cause of hypoventilation.
