Where should the nurse place the chest tube drainage system?

Core Concept: Chest tube drainage systems rely on gravity and a water seal (or one-way valve) to evacuate air and fluid from the pleural space. To prevent the backflow of drainage or air back into the patient's pleural cavity (which could cause a tension pneumothorax), the system must always remain upright and below the level of the chest.
Clinical Presentation:
- Not applicable.
- Complications of improper placement: Fluid reflux into the pleural space, loss of the water seal leading to pneumothorax, or system malfunction.
Diagnosis:
- First-line/Gold Standard: Continuous visual assessment of the chest tube drainage system's position relative to the patient's chest, as well as monitoring the water seal chamber for proper tidaling.
Management:
- Acute: During transport, the drainage system should be placed on the side of the stretcher, securely held or attached to the lower frame, strictly below the level of the patient's chest. It should not be hung on an IV pole or lifted onto the stretcher.
- Definitive: Once at the destination, immediately return the system to its proper stand on the floor, ensuring it remains upright and below chest level.
