A 4 years old girl, was playing outside, she came to her mom crying and holding her right upper arm, she went to the hospital with swelling over the upper arm, pain and itching, the appropriate management is:

Core Concept: The symptoms (swelling, pain, itching after playing outside) strongly suggest a severe allergic reaction (anaphylaxis) to an insect sting (e.g., bee or wasp). In any anaphylactic reaction, the absolute first priority in the ABCs (Airway, Breathing, Circulation) is to assess and maintain a patent airway, as laryngeal edema can rapidly cause fatal asphyxiation.

Clinical Presentation:

  • Localized swelling, pain, pruritus (itching), hives.
  • Severe signs: Stridor, wheezing, dyspnea, hypotension, angioedema of lips/tongue, sense of impending doom.

Diagnosis:

  • First-line diagnostic plan: Clinical recognition of anaphylaxis based on acute onset of skin/mucosal symptoms combined with respiratory compromise or hypotension.
  • Gold standard test: Serum tryptase levels (elevated in anaphylaxis, but results are delayed and do not guide acute management).

Management:

  • Acute management: Maintain airway, administer IM epinephrine immediately, provide supplemental oxygen, and give IV fluids/antihistamines/corticosteroids as adjuncts.
  • Gold standard long-term/definitive management: Prescribe an epinephrine auto-injector (EpiPen), refer to an allergist for venom immunotherapy, and educate the family on trigger avoidance.
