Core Concept: Acute Rheumatic Fever (ARF) is an autoimmune sequela of untreated or inadequately treated Group A Beta-Hemolytic Streptococcus (GAS) pharyngitis/tonsillitis. The immune response cross-reacts with cardiac tissue, joints, and the central nervous system, typically occurring 2-4 weeks after the initial streptococcal infection.

Clinical Presentation:

  • Preceded by sore throat/tonsillitis 2-4 weeks prior
  • Jones Criteria: Migratory polyarthritis, carditis, subcutaneous nodules, erythema marginatum, Sydenham chorea
  • Fever, elevated ESR/CRP, prolonged PR interval on ECG
  • Complications: Chronic rheumatic heart disease, mitral valve stenosis/regurgitation, heart failure

Diagnosis:

  • First-line diagnostic plan: Thorough history focusing on recent pharyngitis/tonsillitis, followed by physical examination using the Revised Jones Criteria (requires 2 major or 1 major + 2 minor criteria plus evidence of preceding GAS infection).
  • Most accurate (gold standard) test: Throat culture for Group A Streptococcus, elevated anti-streptolysin O (ASO) titer, or anti-DNase B antibody levels to confirm recent streptococcal infection.

Management:

  • Acute management: Bed rest during the active inflammatory phase. Administer aspirin or NSAIDs for arthritis, corticosteroids for severe carditis, and a full course of penicillin (or erythromycin if penicillin-allergic) to eradicate residual GAS.
  • Gold standard long-term management: Secondary prophylaxis with intramuscular benzathine penicillin G every 3-4 weeks (or daily oral penicillin) to prevent recurrent streptococcal infections and worsening of rheumatic heart disease. Duration depends on cardiac involvement (minimum 5 years or until age 21, lifelong if severe carditis).
